The Right Approach

For automatic detection of micro-embolic signature in Transcranial Doppler TCD signals many approaches were taken in the past. Commercialized systems included spectral pattern recognition, digital filtering, wavelet analysis, and bi-gating techniques, most of them complex in application, expensive, and notably all of them TCD instrument specific. The new EDS system, developed by Dr. R.W.M. Keunen MD PhD, a Dutch neurologist, applies periodicity analysis to the TCD audio signals. This approach leverages on the fact that insonated TCD audio signals have an irregular or aperiodic frequency pattern and even more so artifacts. In contrast, embolic signature impresses by rather even or periodic frequency content. The periodicity of the frequency content of the TCD audio time series can be assessed by a simple zero crossing counter, yielding a Zero Crossing Index or ZCI. Feeding the information of continuous ZCI and relative intensity of the audio signal to a neural network EDS reliably detects embolic signature and distinguishes artifacts.

Validated Reliability

The excellent reliability of the EDS with an overall accuracy of 96% in comparison to human expert observers was scientifically evaluated and documented in a peer reviewed publication*. The EDS algorithm has a sensitivity of 0.93 and specificity 0.98 in discriminating emboli and artifacts. This may be well above all previously reported algorithms.

Independent of TCD Instrument

The beauty of the EDS though lies in its simplicity: Only a unidirectional, single gated TCD audio signal is needed. A host of different instrument can serve as signal source to the EDS. The EDS operates quite instrument independent. Different devices have been tested and reliably work with the EDS, both in online and offline mode.

Seamless Delica-9 TCD Series Integration

The EDS can smoothly be integrated with the Delica-9 TCD instruments offered and supported by SMT medical. Only one PC is required for both, the TCD and the EDS programs. The operator can switch between applications, adjust the instrument or observe the test while the EDS algorithm detects and records occurring emboli. A concise report is offered objectively documenting the embolic detection session.

Total Portability

The EDS-Delica combination features unrestricted access to the OR, angio suite, or ward. EDS is exclusively represented by SMT medical, the experts with an experience of 20 years in Transcranial Doppler Sonography.
